CONSENT FORM MUST BE FILLED OUT BEFORE YOUR SESSION. SUBMIT BEFORE ARRIVING, OTHERWISE I CANNOT PERFORM THE SERVICE. THIS ALLOWS ME TO ALSO KNOW A LITTLE BIT ABOUT YOUR GOALS, ALLERGIES, HEALTH IN ORDER TO CATER TO YOUR SPECIFIC NEEDS.

DURING SERVICE

MUST WEAR A MASK, WE ARE STILL IN A PANDEMIC AND ITS NOT NICE TO BREATHE ONTO SOMEONE WHO IS WORKING SO CLOSE TO YOUR FACE. I WILL BE WEARING MY MASK FOR YOUR SAFETY, PLEASE THINK ABOUT MY SAFETY AND OTHERS.

RESCHEDULE/CANCELLATION POLICY

NEW APPOINTMENT = NEW DEPOSIT.

LATE MORE THAN 15 MINS: CLIENT WILL HAVE TO RESCHEDULE UNLESS AUTHORIZED TO STILL COME IN. CLIENT WILL NEED TO SEND A NEW DEPOSIT TO HOLD YOUR RESCHEDULED APPOINTMENT.

IF YOU ARE HABITUALLY LATE (MORE THAN 3 TIMES), THERE WILL BE A $20 LATE FEE APPLIED.

FIRST CANCELLATION OR RESCHEDULE WILL BE WAIVED AS CURTESY FOR AS LONG AS WE HAVE KNOWN EACH OTHER. THIS DOES NOT MEAN IT RESETS BY EACH NEW APPOINTMENT MADE.

SECOND CANCELLATION OR RESCHEDULE WILL REQUIRE CLIENT TO PAY 50% OF THE SESSION UP FRONT INSTEAD OF THE $10 DEPOSIT. IT WILL BE NONREFUNDABLE IF YOU DECIDE TO CANCEL OR RESCHEDULE YOUR SESSION AGAIN. CARD ON FILE WILLBE CHARGED 50% OF SERVICE MISSED.

THIRD CANCELLATION OR RESCHEDULE, WILL RESULT IN BEING BLACKLISTED FROM FROM BOOKING AGAIN UNLESS YOU PAY 50% DEPOSIT OF THE SERVICE FOR THE NEW SESSION. IN ADDITION, CLIENT’S CARD ON FILE WILLBE CHARGED 100% OF SERVICE MISSED.

UNCONFIRMED SESSIONS ARE SUBJECT TO BE CANCELLED IF THERE IS NO REPLY TO ANY ATTEMPTS OF BEING CONTACTED AND WILL BE NOTED AS A NO CALL/NO SHOW, THUS RESULTING IN BEING BLACKLISTED.

NO CALL/NO SHOWS: WILL BE CHARGED 100% OF SERVICE SCHEDULED WITH CARD ON FILE. ANY DEPOSIT PUT FOR THIS SESSION WILL BE NONREFUNDABLE AND INAPPLICABLE TO ANY SERVICE.

BLACKLISTED CLIENTS: IF CLIENT HAS BEEN CONSISTENT OR FREQUENTLY CANCELLING, RESCHEDULING, OR DOING NO CALL/NO SHOWS, THIS WILL RESULT IN BEING PLACED IN THE BLACKLIST. IF CLIENT NEEDS TO BOOK CLIENT MUST PAY IN FULL AMOUNT FOR REQUESTED SESSION WHICH WILL NOT BE REFUNDABLE IF THEY DECIDE TO CANCEL/RESCHEDULE. THIS DOES NOT MEAN THE SESSION CAN BE RESCHEDULED, CANCELLED, OR REFUNDED FOR ANY REASON AND WILL NOT BE HELD AS CREDIT ONCE IT HAS BEEN SCHEDULED ON A SPECIFIC TIME/DATE.
